Reimbursement claims Procedure

Original Documents to be submitted in support of claim are For *Reimbursement claims*: from Star Health a. Duly completed claim form, (forms can be downloaded from www.Starhealth.in or get it from any star health office or from us) (form A to be filled in by you, form B to be filled in by the Hospital, for getting Reimbursement 0f Claims) (for getting reimbursment of Pre and post Hospitalisation expenses FORM B is not necessary) and b. Pre Admission investigations and treatment papers in original. c. Discharge Summary in original from the hospital d. Cash receipts in original from hospital, chemists. e. Cash receipts and reports for tests done in original f. Receipts from doctors, surgeons, anaesthetist in original g. Certificate from the attending doctor regarding the diagnosis. h. Copy of PAN Card i. Copy of Aadhaar Card j. Any other document specific to the treatment / illness k. Prescriptions (most of them do not enclose it. pls enclose it so that claim will be settled forit) and receipts for Pre-Hospitalization and Post-Hospitalization Note: The Company reserves the right to call for additional documents wherever required. l. copy of Policy document m. Cancelled Cheque leaf Pls see that all originals required to be submitted. Incase if u need the original Xray, Scan(CT, MRI), for furture reference/Treatments, U can request the Claim depart for return of it , after scrutinising them. We will help and service & do the needful
